TERMS AND CONDITIONS:

• Wellness Packages

  • Packages must be paid in full by Dec 31 2024.
  • Package purchase is not eligible for extended healthcare reimbursement.
  • An invoice will be issued at the time of your scheduled appointment, which can be submitted for extended healthcare reimbursement.
  • Packages cannot be shared or transferred.
  • Funds will never expire.
  • If treatment is no longer needed, refunds can be issued for physiotherapy packages only.
  • Funds can only be applied to services.
  • All prices above include HST (if applicable).
  • Funds can be used immediately.

  • No upfront payment.
  • Credit card will be kept on file for monthly payments.
  • 12 month commitment (December - December).
  • Monthly memberships begin the month of purchase.
  • Monthly Winter Golf Package Simulator Bay Hours will be applied as a credit and can be used at any time during the 12 months.
